Right now, the journal on http://moonfire.us/blog/ uses WordPress 2.1. The extension is Live+Press 0.99 (for WP 2.0, his site is down at the moment). If you use it, I highly recommend you stick with WordPress 2.0 (Live+press 0.99) or 1.5 (the one before 0.99), at least until he fixes the bugs.
As I can tell, as long as a journal place offers an API of some sort, it probably is possible to automatically keep them synchronized. The only thing Live+Press doesn't do is sync comments, but I haven't found ANY place that does that. Though, with OpenID, it might be possible.
If I didn't have Live+Press, I'd probably just write something myself. But, I'm lazy.
Basically, that is how I keep LJ (dmoonfire.livejournal.com) and my site in sync. :) I hope it help.
